Who Is Eligible?
As an employee of Yale-New Haven Hospital, Bridgeport Hospital, Greenwich Hospital, Northeast Medical Group and Yale New Haven Health Services Corporation, you’re eligible for medical, prescription drug, dental and vision care coverage if you’re a full-time or part-time employee regularly scheduled to work at least 24 hours per week.
Additionally, your lawfully married spouse, your civil union partner (entered into in states that do not recognize same sex marriage) and your dependent children are eligible.
You may elect the following types of coverage:
- Employee (yourself only)
- Employee + Spouse
- Employee + 1 Child
- Family (yourself plus two or more family members)
You are required to provide supporting documents to enroll your dependents such as a copy of your marriage certificate to enroll your spouse and birth certificates to enroll your children.

For the medical, prescription, dental, vision and life insurance plans, your dependent children must be under age 26. Your dependent children include your biological children, stepchildren, adopted children, including those placed for adoption, your eligible foster children, and any other child for whom you are responsible pursuant to court order, including a qualified medical child support order.
Coverage terminates at the end of the month when your child reaches age 26.
You may be required to submit reasonable documentation (such as a family court order, custody order, or, if applicable, a 1040 tax form) to verify your child’s eligibility.
Coverage for your fully handicapped dependent child may be continued past the child’s 26th birthday, but only if you have submitted proof that the child is disabled, by no later than 31 days after the date your child reaches 26. Coverage will cease when your child is no longer handicapped, or if you fail to give proof of continued disability, fail to have any required exam or when dependent coverage terminates for any other reason.
