CVS Caremark will provide coverage for women’s contraceptives that require a prescription as follows:

Oral Contraceptives (birth control pills):

• Generic and single-source brands (those without a direct generic equivalent) will be covered at 100%, no copay.
• Multi-source brands that have generic equivalents will be covered subject to member cost share under the pharmacy benefit (see Prescription Drug Coverage).

Other contraceptive methods:

• Barrier methods (e.g., cervical cap, diaphragm, vaginal ring) and devices such as IUDs will be covered at 100% (insertion of IUDs will be covered under the medical plan).

• Select FDA-approved female over-the-counter contraceptives are covered at 100% when accompanied by a prescription.
